West & East

Des Moines Orthopaedic Surgeons has two convenient locations to serve you. Our East office is located at Penn Medical Place in Des Moines, Iowa. 
In West Des Moines the DMOS Orthopaedic Center is located at 6001 Westown Parkway. DMOS also has a dozen satellite clinics to serve our neighboring communities. 

The following forms are to be completed by every new patient that visits Des Moines Orthopaedic Surgeons. To save time at your clinic visit please print and complete the General Health History Form and New Patient Registration Form and bring them to your appointment (they cannot be filled out online). A copy of your insurance card will also be necessary. If your insurance plan requires a physician referral to see a specialist, please make the appropriate arrangements with your primary care physician before your appointment at DMOS.
