How to Complete Your Forms

Click "Get Forms Completed" button below.
Enter your email: You'll receive a secure link to that email to access Request Manager (No username and password to remember – whew!)
Follow the step-by-step instructions on the screen to submit your request.
Medical Records are free. Forms completion is $45 per form. If you need assistance exploring financial support options for form fees please call 800.659.4035. For Status Updates on submitted requests: email fmla@healthmark-group.com or call 972.895.2138
HealthMark will process your requests. You will receive an email with a link to download your records once the request is complete and/or forms are sent to the location indicated in your forms order.

Please note: Requests for FMLA and/or disability forms take an average of 7 business days to be reviewed by the provider. Medical record requests generally take 2-3 business days.
